Pregnancy Planning? Here’s What Your Doctor Wishes You Did 6 Months Before

Pregnancy Planning? Here’s What Your Doctor Wishes You Did 6 Months Before

Most pregnancy advice begins at the positive test – but by then, the most consequential window for preparation has often already closed. The neural tube closes within the first 28 days after conception, often before a woman even knows she’s pregnant. Uncontrolled diabetes, thyroid dysfunction, and nutritional deficiencies affect foetal development from implantation. Preconception care – medical preparation in the months before trying to conceive – is standard practice in evidence-based obstetrics, and it remains significantly underused in India, where care usually starts only after a confirmed pregnancy.

Start With a Preconception Consultation

Book a preconception consultation with a gynaecologist before stopping contraception, not after a positive test. This appointment reviews your full medical history, current medications (some are contraindicated in pregnancy and need switching beforehand), menstrual cycle regularity, and lifestyle factors – diet, exercise, alcohol, and smoking – that affect conception and early pregnancy.

The Preconception Blood Panel

A targeted preconception blood test panel identifies conditions that affect fertility and foetal development, many of which produce no symptoms:

  • Thyroid function (TSH, Free T4) – even mild subclinical hypothyroidism raises miscarriage and preterm birth risk, and the ideal preconception TSH target is lower than the standard ‘normal’ range
  • Blood sugar and HbA1c – organ formation happens in the first eight weeks, so undiagnosed diabetes at conception carries real risk of congenital abnormality
  • Full blood count and iron studies – correcting iron deficiency before conception is far easier than managing it once pregnancy nausea sets in
  • Rubella immunity – the MMR vaccine can’t be given during pregnancy, so non-immune women need it, plus a one-month wait, before conceiving
  • Blood group, Rh factor, hepatitis B/C, HIV, and syphilis screening, and vitamin D levels

Folic Acid – Start It Before You Conceive, Not After

The neural tube closes between days 21 and 28 after conception – before most women have even missed a period. Adequate folic acid (400–500 mcg daily, started at least a month before trying to conceive) reduces neural tube defect risk by roughly 70 percent. Starting it only at the first antenatal visit, around week eight, misses this window entirely. Check the label on your prenatal vitamin – some contain only 200 mcg, below the recommended preconception dose.

Conditions Worth Addressing Before You Try

Women with PCOS often need an ovulation induction plan established before trying to conceive, rather than months of unexplained non-conception. Chronic conditions – diabetes, hypertension, epilepsy, autoimmune disease – often require medication changes that are safer done ahead of pregnancy than in a rush after a positive test. Anyone with a previous pregnancy loss benefits from a preconception work-up for recurrent miscarriage causes, done while there’s time to act on the results.

Don’t Forget the Partner

Male factors account for close to half of all fertility issues. A semen analysis is worthwhile if conception hasn’t occurred within six months of trying. Stopping smoking, reducing alcohol, avoiding prolonged heat exposure to the testes, and reviewing any medications that affect sperm production all matter in the three months before trying – a full sperm production cycle.
